# Person-Centered Planning Tools

[The LifeCourse framework](http://www.lifecoursetools.com/) was originally developed by Missouri Family to Family, housed within Missouri’s University Center for Excellence in Developmental Disabilities Education, Research and Services (UCEDD) at the University of Missouri–Kansas City Institute for Human Development.

The [Integrated Services and Supports](http://www.lifecoursetools.com/principles/integrated-supports/) worksheet is designed to help families and individuals think about how to work in partnership to support their vision for employment.

[County Connections Initial and Annual Assessment ](https://www.lifecoursetools.com/examples/loqw-initial-annual-assessment/)is a very holistic assessment of individuals' network, goals, and needs, based on the LifeCourse categories. It gives a clear picture of any gaps that may be missing from natural and paid supports.

[The Positive Personal Profile](https://employmentfirstma.org/files/PositivePersonalProfile_Transcen.pdf) is a person-centered planning tool used by TransCen and other providers. It takes inventory of all of an individual's positive attributes as well as where they need support.
