The worksheet is designed to help families and individuals think about how to work in partnership to support their vision for employment.

is a very holistic assessment of individuals' network, goals, and needs, based on the LifeCourse categories. It gives a clear picture of any gaps that may be missing from natural and paid supports.

is a person-centered planning tool used by TransCen and other providers. It takes inventory of all of an individual's positive attributes as well as where they need support.
